De grote farmaceutische industrie (big farma) heeft het moeilijk. De industrie moet weer op het spoor komen. Kunnen de nieuwe grootmachten India en China de weg wijzen?<\/strong><\/p>\n

Dit is het tweede van vier artikelen over de farmaceutische industrie.<\/em><\/p><\/blockquote>\n

De farmaceutische industrie verschoof naar India en China<\/strong><\/h2>\n

Tegenwoordig zijn India en China de grote producenten van medicijnen. En dat komt maar ten dele door de lage lonen. In mijn functie bij DSM heb ik dit proces van nabij meegemaakt. Het gaat om de bekende b\u00e8ta-lactam antibiotica; nog steeds de belangrijkste koortsremmende middelen. Het voorbeeld is illustratief voor de ontwikkeling van de farmaceutische industrie in India en China.<\/p>\n

Tegen het eind van de vorige eeuw werd India een grote consument van medicijnen, en later ook China. Voor de hand liggend, met hun grote bevolking, betere gezondheidszorg en verzekeringsmogelijkheden. Maar de import van kant-en-klare geneesmiddelen uit het Westen was voor hen al snel onbetaalbaar. Vanaf ca. 1970 nam de lokale productie van medicijnen een grote vlucht. Eerst door het actieve bestanddeel op de wereldmarkt te kopen en de pillen en poeders lokaal te maken. Al snel gevolgd door het zelf maken van de actieve bestanddelen. De markt van DSM in India en Zuidoost Azi\u00eb groeide heel snel, en we wilden die maar al te graag blijven bedienen vanuit Nederland. De pogingen om grondstoffen en tussenproducten ter plaatse te maken waren dus niet in ons belang.<\/p>\n

Dit plaatste DSM voor een dilemma. Verplaatsing van onze fabrieken naar India zou onze marges aanzienlijk verkleinen. Een eventuele winst zouden we ook nog moeten delen met een (verplichte) Indiase partner. Wij kozen voor ontmoedigen. Maar uiteindelijk gingen anderen toch ons product maken en wij dreigden de hele markt kwijt te raken. Wij zijn toen te laat met een Indiase partner in zee gegaan en moesten later ettelijke miljoenen op tafel leggen om weer mee te kunnen doen. Niet zozeer om in India geld te verdienen \u2013 dat was zeer moeilijk vanwege de moordende concurrentie. Maar om onze overige markten te beschermen. Uiteindelijk werd India toch wereldmarktleider. Onze positie viel terug van wereldmarktleider, via leverancier van proceskennis en fabrieken, tot nul door verkoop van de business. Indiase ondernemingskracht is de grootste reden voor onze afhankelijkheid van medicijnen uit die landen.<\/p>\n

Elke maand een stuk zeep<\/strong><\/h2>\n

Op het eerste oog is het Chinese verhaal heel anders. Tot ca. 1980 was het land op slot voor westerse handel. Maar toen het land open ging bleek het allerlei medicijnen te produceren. Achteraf gezien logisch voor een land met zo\u2019n grote bevolking. Bij de economische openstelling behoorde dat Chinese producenten een klein deel van hun productie konden exporteren; een deel van de dollarinkomsten mochten ze naar eigen inzicht besteden aan importen. Doordat hun kosten al gedekt waren door de lokale afzet, was de internationale prijsstelling van hun farmaceutische industrie vanaf dag \u00e9\u00e9n zeer concurrerend. Van hun kant stonden westerse bedrijven te trappelen om zaken met China te doen. Stel je voor dat we elke Chinees elke maand een stuk zeep kunnen verkopen, dachten ze bij Unilever met grote dollartekens in de ogen. De Chinese industriepolitiek was echter heel helder: importeren mag, maar tijdelijk en tegen tamelijk hoge tarieven. En op termijn moet u met uw kennis en productie naar China komen en samenwerken met een lokale partner. Dat is wat DSM heeft gedaan; de toenmalige partners zijn nu de wereldspelers.<\/p>\n

China en India als de toekomst voor nieuwe medicijnen?<\/strong><\/h2>\n

Zowel de Chinese als de Indiase fabrieken op de internationale medicijnenmarkt zijn FDA goedgekeurd en produceren medicijnen van hoge kwaliteit. China en India hebben beide een sterke farmaceutische industrie ontwikkeld door de kracht van het land zelf. De ondernemingszin van de Indi\u00ebr en de geleide economische politiek van China, met vergelijkbaar resultaat. In beide gevallen geruggesteund door heel grote eigen, lokale markten! India en China samen hebben evenveel fabrieken voor de productie van medicijnen als zowel Amerika als de EU.<\/p>\n

Kunnen India en China dan misschien de rol overnemen van ontwikkelaar van nieuwe medicijnen? Of van bedenker van nieuwe concepten voor de gezondheidszorg? Kansrijk zou je denken, zeker als je ook de eeuwenoude cultuur en de traditionele geneesmiddelen erbij betrekt. Maar vooralsnog is het antwoord kort: nog niets van te zien. Misschien kan Japan tot voorbeeld dienen. Het land heeft een vergelijkbare achtergrond. Zijn farmaceutische industrie is niet veel verder gekomen dan slim kopi\u00ebren van EU en USA; de ontwikkeling van nieuwe medicijnen heeft maar beperkt succes gehad. Alles volgens het westerse model.<\/p>\n
