Grote farmaceutische bedrijven (big farma) hebben het moeilijk. Aandeelhouders klagen over dalende winstmarges; overheden, verzekeringsmaatschappijen en pati\u00ebnten klagen over torenhoge medicijnprijzen. De industrie moet een nieuwe weg vinden \u2013 maar welke?<\/strong><\/p>\n

Dit is het eerste van vier artikelen over de farmaceutische industrie.<\/em><\/p><\/blockquote>\n

Het bedrijfsmodel van big farma is verouderd<\/strong><\/h2>\n

In de tweede helft van de vorige eeuw kende big farma gouden tijden. Veel bedrijven schreven dubbele winstcijfers. Ook in termen van verbetering van de volksgezondheid waren de prestaties indrukwekkend: vitamines, antibiotica, cardiovasculaire preparaten, anti-aids medicijnen, anti-reumamiddelen enz. Maar tegen het eind van de eeuw begon de klad erin te komen. Het vinden van nieuwe middelen werd steeds moeilijker en duurder. En de toegevoegde waarde voor de volksgezondheid kwam onder vuur te liggen. Denk aan de cholesterolverlagers. Biotechnologie en gentherapie beloofden veel, maar resultaten bleven almaar uit; ontwikkelkosten werden alleen maar hoger. Er ging meer geld naar juristen om bestaande posities te handhaven dan naar het vinden van nieuwe middelen. Ranitidine tegen maagzweren van GSK is een notoir voorbeeld. De moraal werd steeds kapitalistischer.<\/p>\n

Tijd om de bakens te verzetten. Maar het is nog steeds niet duidelijk welke kant het op zal gaan. Het ontwikkelen van nieuwe medicijnen in de eigen, vaak heel grote laboratoria is minder succesvol en kost meer dan vroeger. De focus ligt nu op het uitkopen van eenlingen (Willy Wortels) en kleine gedreven ondernemingen. Die doen nu vaak de meest beloftevolle ontdekkingen. Maar ze hebben bij benadering niet het geld om hun succesvolle vinding te loodsen door de vele veiligheidstests, en ook nog eens te marketen. Dat doet big farma wel voor hen. Want de totale ontwikkelkosten zijn v\u00e9\u00e9l groter dan die van de uitvinding. Vooral door de steeds strengere veiligheidseisen die maatschappij en overheden stellen. Nu er een wedloop op gang is gekomen naar een coronamedicijn vangt de samenleving daar een glimp van op. De meeste Willy Wortels varen er trouwens w\u00e9l bij. Veel kleine uitvinders zijn nu ook miljonair.<\/p>\n

Peperdure geneesmiddelen<\/strong><\/h2>\n

Eind vorige eeuw ontdekte big farma dat marktprijzen geheel onafhankelijk kunnen zijn van kostprijzen. Marktprijzen worden bepaald door wat klanten voor het product over hebben. Of door wat de maatschappij \u2018redelijk\u2019 vindt voor een gewonnen levensjaar. En voor hun gezondheid hebben mensen heel v\u00e9\u00e9l over. In de coronacrisis hebben we zelfs de economie ervoor stilgezet. Gezondheid heeft geen prijs, heeft big farma ontdekt. En met hen nog een hele reeks bedrijven die zich op gezondheid richten, denk aan Philips. Maar big farma heeft de meeste ruimte. Niet dat ze alles naar hun hand kunnen zetten. Hun kosten zijn onvoorstelbaar hoog, ook door onze eigen wetten en regels. Ze kunnen zich nergens verzekeren. De productaansprakelijkheid komt helemaal op hun eigen bordje. De gouden tijden van eind vorige eeuw zijn voorbij. Ruzie dus in de boardrooms en op aandeelhoudersvergaderingen. Zou de wedloop naar een middel tegen corona ons een nieuw model kunnen opleveren? Ik vrees van niet. Misschien laat het op de meest g\u00eanante wijze zien dat we beter moeten gaan nadenken. Vooral ook over de grenzen van de maakbaarheid van het leven.<\/p>\n

Zouden we geneesmiddelen dan op een andere manier kunnen gaan ontwikkelen? De historie stemt ons niet hoopvol. Ik ken geen succesvolle voorbeelden. De Sovjet-Unie ontwikkelde helemaal niets. Finland met zijn bijzondere politieke positie tussen Oost en West verdiende een tijdlang een fortuin met de doorvoer van westerse middelen. In India heeft de overheid het geprobeerd. Het werd een Walhalla van duur betaalde banen voor allerlei wetenschappers met als uitkomst: nul nieuwe medicijnen. In Nederland is TNO ooit een rol toebedeeld in door de overheid geleide ontwikkeling van medicijnen. Nooit resultaten gezien en een stille dood gestorven. Betere regulering dan van big farma? Weinig kansrijk. Dan moeten we industrie-breed de misstanden aanpakken; te beginnen bij de top. Hebben pogingen om het gedrag te veranderen van Amazon, Google en Microsoft ooit iets opgeleverd?<\/p>\n
